Initial Comments:
An off-site follow up survey was conducted on 2/3/23, and found H3 Care, Inc., had corrected the deficiencies cited under the requirements of 28 Pa. Code, Part IV, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. Deficiencies were cited as a result of a home care agency/registry state re-licensure survey conducted on 1/18/23
